How to Request Your Health Care Records
Clients in Texas, Arizona, and Montana have the right to access their clinical records. To request your records, please follow these steps:
Submit a Written Request: Email your request to hello@storiedsoulstherapy.com or send a secure message via your Carepatron client portal.
Verification: We will verify your identity before releasing any records to protect your privacy.
Timeline: In accordance with Texas law, we will respond to your request within 15 business days.
Fees: Reasonable, cost-based fees for copying or postage may apply as permitted by state statute.
